Community Health Improvement Plan

Launched in January 2016, the Community Health Improvement Plan is the result of a 12-month effort to identify the community health priorities and develop evidence-based strategies to address them. More than 35 organizations from all sectors of the community are actively collaborating to advance the plan’s objectives.

The Council selected later life quality, substance use disorder (including tobacco) and obesity/physical activity as key priority areas and recognizes that poverty and transportation are underlying themes across each of these areas that need attention.  Three sub teams, each with a team champion, were formed to develop and maintain progress of the action plans.  One to three objectives were selected for each priority area in order to maintain focus, improve progress and increase the chances of long-term success.

The Community Health Improvement Plan team will continue meeting every other month and a progress update will be posted to this website on a quarterly basis. Questions about the plan should be directed to Sharon Mortensen at (989) 839-9661.
